Evanston Public Library - Medical Marijuana Sign Up Event

Join us as we discuss the Compassionate Use Act of Medicinal Cannabis Pilot Program set forth by the Illinois Department of Public Health. This event is free and open to the general public. Friends and family are welcome! To RSVP and save a seat at our next seminar, we encourage you to send your RSVP to [email protected] Those who have questions, want to learn more, or are in need of obtaining a medical cannabis card are welcome to attend. PharmaCannis will offer attendees the information they will need, and an opportunity to apply to the State's program onsite. Applying for the program however, is not a requirement to attend this event. If one decides to apply on-site, please have the following required documents in hand: Qualifying Patients: -State ID -Two legal proofs of IL residency -Check payable to the Illinois Department of Public Health in one of the following amounts: $100 (1 year term selected); $200 (2 year term selected); $250 (3 year term selected) Caregivers: -State ID -Two legal proofs of IL residency -Check payable to the Illinois Department of Public Health in one of the following amounts: $25 (1 year term selected), $50 (2 year term selected) or $75 (3 year term selected) if applying at the time of the patient; $100 if applying separate time from initial patient application Qualifying Veterans: -State ID -Two legal proofs of IL residency -VA Form 10-5345 (obtained by PharmaCannis) -DD214 Discharge Form -Check payable to the Illinois Department of Public Health in one of the following amounts: $50 (1 year term selected); $100 (2 year term selected); $125 (3 year term selected) Patients with Social Security Disability Income or Supplement Security Income: -State ID -Two legal proofs of IL residency -Social Security Benefit Verification Letter (obtained by calling: (800-772-1213) -Check payable to the Illinois Department of Public Health in one of the following amounts: $50 (1 year term selected); $100 (2 year term selected); $125 (3 year term selected) PLEASE NOTE: The event itself is free and always will be. The fees are solely for the State application. Please bring a check that will be made out to the Illinois Department of Public Health. Additionally, there is an option to have finger printing and your 2 x 2 photo taken onsite at the event with a 3rd party vendor, at a separate and additional rate of $55-$65 (fingerprints) and $5 (photo). Cash, check and credit cards are accepted by the finger print vendor.
