Midway Dispensary hosts weekly events to raise awareness about the Illinois medical cannabis pilot program, and to help with your application, fingerprints and photos required to apply. We will answer all your questions about the program during our application assistance sessions. To find out what you need to bring, please visit www.midwaydispensary.com. Make sure you get your physician to certify your qualifying condition and have your physician submit the form to the IDPH. Forms are available at https://midwaydispensary.com/become-a-patient/. If you are a veteran, we will need the last full year of records, and your DD214 from the VA to complete your application.
