Thank you for considering being a part of our HealthFORCE event! Though you will be invoiced later, completion of this registration form means you are signed up to attend the Friday October 4th event! After you complete & submit, someone from The Health Collaborative will contact you by e-mail with an attached copy of an invoice. You can also contact us immediately if you have questions or concerns. Set-Up & Tear Down Parking is FREE. (You may use loading area for both set-up & tear-down.) Set-up time is 8 am but you may begin set-up anytime 7:45 or after and must be set-up by 8:45. Tear Down may begin at 1:30 pm. Schedule Session 1 (1st wave of students) - 8:45 am - 10:45 am LUNCH/BREAK (exhibitors only) - 10:45 am - 11:30 am Session 2 (2nd wave of students) - 11:30 am - 1:30 pm Booth Cost One booth: THC Member: $250 Non-Member: $350 Two booths: THC Member: $500 Non-Member: $625 Three Booths: THC Member: $700 Non-Member: $850 Four Booths: If you book three booths for HealthFORCE, you have the option of adding a fourth booth for only $50! Additional Booth Info Each booth will automatically receive 2 chairs and free wi-fi. Each booth can also receive free electrical (power outlets for devices) to use upon request. It is suggested you bring your own table coverings - however we will provide a basic covering for every booth. Included are 2 lunches/brunches per table. If you will need more, please let us know. Please note, when choosing a profession to exhibit, your booth should: Simulate the Work Environment of the chosen profession (be creative - you may want to use such props as a mannequin, occupation-specific equipment, tools, photos, etc.) Provide a "Hands-on" Activity to engage students (i.e. lab slides under a microscope, take blood pressure, use an oximeter, games, tools they can touch, etc.) OR if you are unable to think of an activity, you may provide up to date information regarding your programs. You can use table signs behind or on top of your table.
